Diagnostic utility of Computed Tomography Angiography (CTA) in the triage of patients with acute chest pain of suspected cardiac origin: An overview and a critical appraisal of a recently conducted systematic review and meta-analysis

چکیده

The current text provides a brief overview and a methodological appraisal of the systematic review and metaanalysis conducted by Samad and colleagues (2012) who assessed the clinical utility and diagnostic accuracy of CTA for the diagnosis of acute coronary syndrome (ACS) in low to intermediate risk patients presenting to the emergency department (ED) with acute chest pain of suspected cardiac origin. In order to assess the methodological quality of the review, we retrieved the original papers and carefully examined the primary data and the review process using a number of methodological assessment tools such as the QUADAS 2, the STARD and an adapted version of the PRISMA checklists. Here we report on the results from our appraisal and discuss, taking into consideration the findings from previous meta-analyses, whether the presented evidence is sufficient and reliable enough to enable policy decisions and inform changes in the current clinical practice.

A meta-analysis and systematic review of computed tomography angiography as a diagnostic triage tool for patients with chest pain presenting to the emergency department.

BACKGROUND To assess clinical utility of computed tomography angiography (CTA) in the diagnosis of chest pain patients presenting to emergency departments (EDs), we conducted a meta-analysis of CTA in patients with suspected acute coronary syndromes (ACSs). METHODS 386 studies were identified on initial review of literature. Inclusion criteria were: (1) prospective study with ≥1 month follow-...

Cardiac Metastasis of Breast Cancer: A Case Report

Breast cancer is one of the most common cancers in women. As we know, cardiac metastases occur rarely. In this study, a 40-year-old woman referred to the hospital due to severe pain in the right leg. She was diagnosed with deep vein thrombosis (DVT) and received treatment, but, after one month and a half she returned with severe pain, exacerbated shortness of breath, cramping pain, and chest pa...

Current Concepts in Cardiac CT Angiography for Patients With Acute Chest Pain

This article presents specific examples of delayed diagnosis of acute coronary syndrome, acute aortic dissection, and pulmonary embolism resulting from evaluating patients with nonspecific acute chest pain who did not undergo immediate dedicated coronary CT angiography (CTA) or triple rule-out protocol (TRO). These concrete examples of delayed diagnosis may advance the concept of using cardiac ...

Coronary CT in Acute Cardiac Care

Background: In this prospective study we determine the diagnostic value of coronary CT angiography (CTA) and calcium imaging in low to intermediate risk acute chest pain patients. Methods: One hundred and eleven consecutive patients (57±11 years, 71 males) presenting to the emergency department with chest pain suggestive of acute coronary syndrome (ACS), but without indication for immediate cat...
